The film tells the story of Lan Zuo, a Gu master of the Tian-Gu Sect, who tumbled into an unknown cave while fleeing in Miao territory in the early 1970s. Many years later, on the night of a blood moon that occurs once in a thousand years, Lan Zuo's former comrade Liu Hong kidnaps eight women of extreme yin energy to refine "living human Gu," attempting to resurrect the ancient Chi You and restore the Xue-Gu Sect to its former glory and fame across the world. Just then, Lan Zuo, who had leapt off a cliff years ago, reappears in the world, rejuvenated into a "fresh-faced young man." Joining forces with his wife Lan Yi, who hides her identity as a Miao doctor, and the rebellious college student Lan Xiang-Ting, he faces Liu Hong in an epic battle of the Insect Master against a monstrous form, to stop this catastrophe from befalling the world.
